The Medication Therapy Management Program (MTMP) is a service offered by HealthPlus to Medicare members who meet a set of specific criteria to ensure their medications are working and to help identify and reduce possible medication problems.
This program is not a Medicare benefit, it is a free service HealthPlus provides to help you and your doctor make sure that your medications are working to improve your health.
To qualify for the program, you must meet all of the following criteria:
If you meet criteria you will receive a letter to invite you to participate. The letter will provide an explanation of the program and
give you the opportunity to decline the service if you do not wish to participate. The introductory letter will also offer you the opportunity to request a Comprehensive Medication Review (CMR).
A CMR is a discussion and review of all of your medications by a pharmacist or other health professional to help you use your medications safely. This is done at no cost to you. This review is provided via telephone by SinfoniaRx, Inc., which provides this service on behalf of HealthPlus. SinfoniaRx, Inc.'s services are provided by licensed pharmacies operated by the University of Arizona College of Pharmacy Medication Management Center and the Ohio State University College of Pharmacy Medication Management Program. You will get a written summary of this call to help you get the most from your medications. You can take this summary with you when you talk with your doctors. Additional services are offered to both you and your doctors, which are aimed to improve your health.
