    HealthPlus and PDC Benefit Program, Inc.

    The PDC Benefit Program, Inc. is a Multiple Employer Welfare Arrangement (MEWA) that encompasses affiliates of the following physician organizations:

    • Oakland Physician Network Services (OPNS)
    • Mercy Physician Community PHO (M-PC PHO)
    • Huron Valley Physician (HVPA)
    • Livingston Physician Organization (LPO)
    • Greater Macomb Physician Hospital Organization (GMPHO)

    The arrangement provides the possibility of medical cost savings for its members.

    PDC Benefit Program Coverage Options

    How do I get started?

    1. Download the Enrollment Paperwork and sign the Participation Agreement:  
    2. Work with your agent to answer any questions you may have about the program. If you do not have an agent, email our PDC Benefit Program team to request an agent assignment.
    3. Select your plan(s) of choice; you may include dental and/or vision riders.
    4. Once you've chosen your coverage, schedule an open enrollment meeting with your employees to review the plan options and have each participating employee complete an Enrollment Application.
    5. Submit all required enrollment documentation to your HealthPlus Account Executive or to the PDC Benefit Program Team.
