Physicians receive patient specific care profile reports on their patients who have chronic conditions such as: Asthma, Chronic Obstructive Pulmonary Disease, Cardiovascular Conditions, Diabetes, and Heart Failure. These reports identify patients who have certain co-morbid conditions, such as hypertension or chronic kidney disease and provide information related to laboratory claims and medications. Reports are placed in the Provider Report Portal twice per year, for physicians in the HealthPlus network , who have five or more patients that fall into the chronic care categories identified above.
HealthPlus generates a Chronic Care Profile Report on each HealthPlus member, in the HealthQuest Disease Management Program, who has been identified with one or more chronic diseases, based on medical and pharmacy claims data. The profile will include services incurred and processed for the rolling 12 months, prior to the report date.
