Effective midnight on December 29, 2015 the online agent portal will no longer be available for Signature Individual plans. All changes to existing policies will require the use of an individual change request form signed by the member. Forms may be found on the new Forms and Downloads page.
During 2016, any qualifying events (SEP) for existing policies will need to be processed by sending an email to IndSales@healthplus.org with all required documentation attached. For your convenience, please refer to the SEP Qualifying Event chart.
