If you are one of those members and want to quit, HealthPlus is here to help you. The HealthPlus Partners Quit Tobacco Coaching Program pays you to quit! We will award three levels of gift cards to those HealthPlus Partners members who attempt and succeed at quitting tobacco.
How Does it Work?
- Enroll in the Program
When you enroll and complete the first call in the HealthPlus Quit Tobacco Coaching program you will receive a $50 gift card.
- Complete the Program
When you complete the entire HealthPlus Quit Tobacco Coaching program you will receive a $50 gift card.
- Quit Tobacco
When you submit a negative cotinine lab test (ordered from your doctor), you will receive a $100 gift card. This lab slip must be received at least six months after you start of program but no more than 12 months from your start date.
Completion is defined as a member being active (online and/or telephone) with their health coach for the entire tobacco coaching program. Currently this program is a five-call model that should last about three months. Being active is defined as having three interactions (online and/or by telephone) each of the three months of the program. Participants must remain a HealthPlus Partners member through the whole of the tobacco coaching program, or they will not receive their reward(s).
Cotinine is an alkaloid found in tobacco and is also a metabolite of nicotine. HealthPlus Partners members are only eligible to participate in this incentive program once during a calendar year.
This program is effective November 1, 2011. Gift card awards will be mailed to recipients within 4-6 weeks of completion of each step outlined above.
