Eligibility
Signature plans are available to children and adults through age 64. Only adults age 18 to 30 are eligible for the Signature One plan. Family policies are available to adults and their dependents age 18 and younger. Those wishing to provide health coverage for children age 18 and older may purchase additional individual policies for those children.

Signature PPO plans are offered in Arenac, Bay, Clare, Clinton, Eaton, Genesee, Gladwin, Gratiot, Huron, Ingham, Iosco, Isabella, Lapeer, Livingston, Macomb, Midland, Montcalm, Oakland, Saginaw, Sanilac, Shiawassee, St. Clair, Tuscola, Washtenaw, and Wayne Counties.
