The HealthQuest Asthma Management Program is our approach to helping people with asthma manage their asthma.  HealthPlus provides support and educational materials to you and your doctor.  If you are eligible for this program, you and any dependent family members with asthma will be automatically enrolled to participate at no additional cost to you as part of your health plan's services

• Free quarterly newsletter for people with asthma
  
• Self-Care Reminder Letters – Regular reminders of important screenings, immunizations and tests to encourage you to take advantage of preventive care services.
  
  

How to enroll in the program

Members must qualify for the HealthQuest Asthma Management Program by meeting criteria established by HealthPlus of Michigan.

To enroll or request more information call 1-800-345-9956, extension 1943, option 4, then option 1 for asthma.  or email dismgmt@healthplus.org

To opt out of the program, call 1-800-345-9956, extension 3246, and leave your name, phone number, member ID, and program name, or email  dismgmt@healthplus.org

Physicians may enroll a member in the program and/or receive additional information about the asthma program by contacting HealthPlus at 800-332-9161, ext. 8944, to speak with a Disease Management Coordinator, or by e-mailing dismgmt@healthplus.org.
